In applying for the loan, I/We have or will complete a loan application containing various information on the purpose of the loan, employment, property, land, taxes and income information, and assets and liabilities. I/We certify that all the information provided is true and complete. I/We made no misrepresentation on the loan application or any other documents, nor did I/we omit any pertinent information that is relevant to this loan.

I/We understand and agree that the Lender reserves the right to verify the information we have provided on the application with my employer, court, my business and/or any financial institution.

I/We fully understand that it is a Federal crime punishable by fine or imprisonment, or both, to knowingly make any false statements when applying for a mortgage loan, as applicable under the provisions of Title 18, United States Code, and Section 1014.


To Whom It May Concern:
I/We have applied for a loan/financing/mortgage from: through their application process and the Lender may verify any information contained in my/our loan application and in other documents required in connection with the loan. It may be verified either before the loan is closed or at the time of the Lender's quality control process.

I/We authorize you to provide to the Lender listed above, the Lender's investors to whom the Lendermay sell my mortgage loan, to provide any and all documents that they request. Such information includes, but is not limited to, employment history, income; bank, money market accounts, personal taxreturns, property information and similar account balances; this may also include my credit history; andcopies of my business and corporate income tax returns.

The Lender, or any other investor that purchases the mortgage loan, may address this authorization to any party named in the application. Each principal partner must complete a copy of this form and sign it, if you are signing for the loan as a individual or as a company.
